Community Health Worker

Job Summary

A company is looking for a Community Health Worker to join their Case Management team.

Key Responsibilities

• Assist members in following an outcome-based plan of care and address current needs
• Outreach to identified members to solve barriers affecting their care
• Monitor care and services to ensure quality care delivery in appropriate settings

Required Qualifications

• High school diploma or GED
• CHW certification from an approved Community Health Certification Program or two years of equivalent work experience
• 1+ years of experience in care coordination and navigation
• Bilingual in English and Spanish or Creole is a plus
• AA or Bachelor's degree in social science or health-related field is a plus
