Job Summary
A company is looking for a Community Health Worker to join their Case Management team.
Key Responsibilities
- Assist members in following an outcome-based plan of care and address current needs
- Outreach to identified members to solve barriers affecting their care
- Monitor care and services to ensure quality care delivery in appropriate settings
Required Qualifications
- High school diploma or GED
- CHW certification from an approved Community Health Certification Program or two years of equivalent work experience
- 1+ years of experience in care coordination and navigation
- Bilingual in English and Spanish or Creole is a plus
- AA or Bachelor's degree in social science or health-related field is a plus
