Compact Licensed Clinical LVN

Job Summary

To support a growing Chronic Care Management team, the full-time Compact Licensed Clinical LVN will conduct high-volume outbound patient calls, manage inbound telephonic interactions, and collaborate with healthcare providers, all while working remotely in a call center environment.

Key Responsibilities

• Manage a combination of inbound and outbound patient calls, maintaining a call volume of 70 to 90 calls per day
• Conduct and document monthly Chronic Care Management care plan calls to ensure comprehensive care coordination
• Collaborate with providers to facilitate timely patient care and accurately document all interactions in real time

Required Qualifications

• Active LPN/LVN Compact License required
• Strong communication and organizational skills
• Proficient with computers, EMRs, and telehealth tools
• Passionate about patient care with the ability to relate with empathy and compassion
• Experience in a call center or high-volume phone-based work environment preferred
