Compact Licensed LPN

Job Summary

To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Compact Licensed LPN will provide education, conduct phone consultations, and maintain patient records while working remotely from anywhere in the United States.

Key Responsibilities:

• Complete phone consultations with patients, offering support and education about their chronic conditions
• Educate patients on health management strategies and assist them in achieving goals aligned with care plans
• Maintain accurate patient records and document all interactions as per protocol

Required Qualifications:

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and a HIPAA-compliant work area
• Empathy and a patient-centered approach to care
