Job Summary
Working remotely, the full-time Behavioral Health Licensed Practical Nurse (LPN) Transfer Center will screen and coordinate patient admissions and transfers, utilizing clinical decision-making to ensure appropriate placements while collaborating with healthcare teams across various facilities.
Key responsibilities
- Screen and coordinate patient admissions and transfers from hospitals and clinics
- Collaborate with intake teams, physicians, and nursing staff to facilitate timely placements
- Monitor patient flow and track delays to improve access processes for better outcomes
Required qualifications
- Active Compact LPN License
- 3+ years of clinical experience in Behavioral Health or Emergency Department/Critical Care
- High-speed internet access (minimum: 50 Mbps upload / 100 Mbps download)
- Demonstrated knowledge of regulatory requirements, including EMTALA
- Commitment to patient-centered care and excellent communication skills
