Job Summary
Working remotely in a full-time capacity, the Behavioral Health Licensed Practical Nurse (LPN) Transfer Center will screen and coordinate patient admissions and transfers, collaborate with healthcare teams to ensure timely placements, and monitor patient flow to enhance access to care.
Key responsibilities
- Screen and coordinate patient admissions and transfers from hospitals and clinics
- Collaborate with intake teams, physicians, and nursing staff to facilitate timely placements
- Monitor patient flow and track delays to improve access processes and outcomes
Required qualifications
- Active Compact LPN License
- 3+ years of clinical experience in Behavioral Health or Emergency Department/Critical Care with Behavioral Health patients
- High-speed internet access (minimum: 50 Mbps upload / 100 Mbps download)
- Demonstrated knowledge of regulatory requirements, including EMTALA
- Commitment to patient-centered care and excellent communication skills
