Compact RN Virtual Care Manager

Job Summary

To support its Virtual Care Platform, the full-time Compact RN Virtual Care Manager will provide critical remote patient support, including care plan development, medication reconciliation, and communication with clinicians, all while working from home.

Key responsibilities

• Provide remote care management support for all virtual care services
• Develop and manage disease-specific care plans, including medication reconciliation and patient education
• Respond to critical escalations and coordinate changes in patient care as necessary

Required qualifications

• Compact RN licensing required or willingness to obtain it
• Preferred experience in cardiology
• Preferred experience in remote care management
• Strong patient assessment skills for effective remote support
• Ability to build meaningful relationships with patients for enhanced healthcare support
