Compact State Licensed Registered Nurse
Job Summary
Seeking a Compact State Licensed Registered Nurse for a full-time remote position, the RN will manage medically complex patients, prepare Annual Wellness Visit charts, and coordinate care while utilizing telehealth workflows.
Key Responsibilities
- Prepare comprehensive Annual Wellness Visit charts and communicate findings to providers
- Conduct telephonic care management and develop individualized care plans for patients with chronic conditions
- Identify and close care gaps while coordinating care across multiple services and providers
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in a compact state
- Minimum 3 years of clinical nursing experience in settings such as bedside, Primary Care, or Home Health
- Experience in telehealth or virtual care coordination
- Proficiency with electronic health record systems and telehealth platforms
- Strong clinical assessment and care coordination skills
