Compact State Licensed Registered Nurse
Job Summary
To support a growing virtual care team, the full-time Compact State Licensed Registered Nurse will manage medically complex patients, develop care plans, and prepare Annual Wellness Visit charts while working remotely on a day schedule that includes weekends.
Key responsibilities
- Prepare comprehensive Annual Wellness Visit charts and conduct virtual patient assessments
- Provide telephonic and portal-based care management, developing and monitoring individualized care plans
- Identify and close care gaps, coordinating care across multiple services and providers
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in a compact state
- Minimum 3 years of clinical nursing experience, including telehealth or virtual care settings
- Experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM)
- Proficiency with electronic health record systems and telehealth platforms
- Ability to work independently in a remote environment with a dedicated workspace
