Complex Care Physician

Job Summary

Providing high-quality, compassionate care remotely, the Complex Care Physician will manage interdisciplinary case conferences, conduct virtual clinical visits, and collaborate with a care team to develop individualized care plans for members with complex medical needs.

Key responsibilities

• Lead interdisciplinary case conferences to guide decision-making on complex cases
• Provide virtual urgent or same-day clinical visits, utilizing evidence-based care for assessment and triage
• Partner with the care team to create and adjust individualized care plans based on members' evolving needs

Required qualifications

• Board-certified physician in internal medicine or family medicine, with interest in complex comorbid conditions and cancer management
• Unrestricted medical license and willingness to obtain additional state licenses as needed
• Experience in virtual or telehealth care delivery and managing medically complex patients
• Comfortable using technology in practice, including video visits and electronic health records
• Experience working with interdisciplinary care teams and supporting shared decision-making
