Connecticut Licensed Intake Nurse

Job Summary

To support home care services, the per diem Connecticut Licensed Intake Nurse will receive and interpret patient referrals, coordinate admissions, and maintain relationships with referral sources in a fully remote capacity.

Key responsibilities

• Receives and interprets referrals from various sources, ensuring clarity in diagnosis and service requests
• Coordinates with nursing and therapy managers to schedule patient admissions and enters referrals into the agency EMR
• Verifies insurance information and prepares monthly referral activity reports as needed

Required qualifications

• Associates or Diploma in Nursing required; BSN in Nursing preferred
• Minimum of five years of nursing experience, with at least three years in home health care preferred
• Valid (or eligible) Connecticut Registered Nurse license required
• Knowledge of insurance, managed care, PDGM, OASIS, and home care eligibility requirements
• Strong computer skills and ability to work effectively with diverse healthcare personnel
