Connecticut Licensed Intake Nurse RN

Job Summary

Receiving and interpreting patient referrals, the full-time Connecticut Licensed Intake Nurse RN will ensure appropriateness for home care services while coordinating admissions and verifying insurance information in a remote work environment.

Key responsibilities

• Receives and interprets referrals from various sources to assess diagnosis clarity and service requests
• Coordinates with clinical staff to schedule admissions and enters referrals into the agency's EMR
• Develops strong working relationships with internal teams and external referral sources while providing excellent customer service

Required qualifications

• Associates or Diploma in Nursing required; BSN preferred
• Minimum of five years of nursing experience, with at least three years in home health care preferred
• Valid (or eligible) Connecticut Registered Nurse license required
• Knowledge of insurances, managed care, PDGM, OASIS, and home care eligibility requirements
• Strong computer skills and ability to work effectively with diverse healthcare personnel
