Job Summary
Providing telephonic care and support, the part-time Connecticut or Rhode Island Licensed Nurse Practitioner will manage patient interactions, coordinate care, and deliver quality health services remotely, with a commitment of 20 hours per week including evenings, nights, and rotating weekends.
Key responsibilities
- Deliver telephonic and virtual care, coordinating with patients, caregivers, and healthcare facilities
- Conduct comprehensive assessments and document medical findings in compliance with regulations
- Facilitate care transitions and monitor patient needs, adjusting care plans as necessary
Required qualifications
- Active and unrestricted Nurse Practitioner license in Connecticut or Rhode Island
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Board certification as a Nurse Practitioner in a relevant specialty
- Current DEA licensure or ability to obtain post-hire
- Minimum of 3 years of clinical experience as an Advanced Practice Clinician (APC)
