DC Licensed Diabetes Nurse Practitioner
Job Summary
To support an innovative telehealth program, the full-time DC Licensed Diabetes Nurse Practitioner will manage patient care through a structured 14-week program, conducting virtual visits, adjusting medications, and developing personalized care plans for individuals with diabetes.
Key responsibilities
- Conduct weekly follow-up visits and provide education to patients living with diabetes
- Collaborate with a multidisciplinary team to deliver coordinated, patient-centered care
- Assess, diagnose, and treat patients, adjusting medications and interpreting glucose data as needed
Required qualifications
- Minimum of 1-2 years of diabetes management experience as an advanced practice provider
- Experience with continuous glucose monitoring (CGM), insulin pumps, and automated insulin delivery (AID) systems is highly desirable
- CDCES and/or BC-ADM certification preferred but not required
- Strong clinical judgment and critical thinking skills
- Exceptional interpersonal and telehealth communication skills
