Electronic Referral Specialist

Job Summary

To support the admissions workflow, the full-time Electronic Referral Specialist will monitor and manage patient referrals from various electronic platforms, ensuring timely follow-up and accurate documentation while working remotely.

Key Responsibilities:

• Monitor multiple electronic referral platforms for new patient referrals and process them promptly
• Maintain comprehensive documentation of referral-related activities and communicate effectively with internal and external stakeholders
• Prioritize and manage multiple referrals simultaneously, escalating complex cases as needed

Required Qualifications:

• High school diploma or equivalent required
• Minimum of 1 year of customer service or healthcare-related experience, preferably in home health
• Familiarity with electronic referral systems such as CarePort, EPIC, or similar platforms is highly desirable
• Ability to type at least 30 words per minute and proficiency in Microsoft Office applications
• Must be fluent in reading, writing, and speaking English
