Job Summary
To support a new provider organization going live in Epic, the part-time Epic Healthy Planet Architect will design end-to-end workflows for population health, ensuring effective data integration between payer and provider systems while advising on best practices and supporting go-live readiness remotely.
Key responsibilities
- Assess and design future-state processes for population health and care gap closure
- Architect bidirectional workflows to ensure actionable payer data integration in Epic
- Partner with integration teams on FHIR activation and advise leadership on operational best practices
Required qualifications
- 5+ years of Epic experience with a focus on Healthy Planet
- Active Epic Healthy Planet certification
- Hands-on experience in registry build and care gap management
- Proven workflow design experience across ambulatory and population health
- Strong understanding of provider operations within Epic
