Job Summary

Supporting patients across brain and mental health services, the full-time Florida Licensed Care Coordinator will manage patient intake, treatment coordination, and ongoing follow-up in a remote setting.

Key Responsibilities

• Conduct patient screening calls and assist with intake processes, ensuring accurate documentation and risk assessment
• Coordinate treatment planning and appointment scheduling while collaborating with clinical teams and external partners
• Maintain compliance with documentation standards and promote a patient-centered approach throughout the care journey

Required Qualifications

• Compliance with all state board requirements, including fingerprinting and application submission
• Proficiency with telehealth platforms and remote clinical workflows
• Experience in a mental health clinic or medical office with an understanding of patient workflows
• Experience in a fast-paced call center environment requiring immediate follow-up
• Ability to work independently and collaboratively within an interdisciplinary team
