Job Summary
To support patients and families affected by chronic conditions related to aging, the part-time Florida Licensed Care Navigator will provide virtual biopsychosocial support, personalized care planning, and community resource connections while collaborating with a transdisciplinary clinical team.
Key responsibilities
- Act as the primary point of contact for caregivers, families, and patients, enhancing quality of life and safety in the home
- Conduct biopsychosocial assessments and develop personalized care plans that align with clients' values and needs
- Coordinate with primary care physicians and healthcare specialists to ensure continuity of care and provide caregiver education
Required qualifications
- Two years of experience in a healthcare or community-based setting preferred
- One year of experience related to the needs of family caregivers and older adults is ideal
- Demonstrated understanding of cultural humility and effective communication with diverse populations
- Familiarity with Electronic Health Records (EHRs) is a plus
- Proficiency in another language, other than English, is a plus but not required
