Job Summary
To support a growing behavioral health practice, the full-time Florida Licensed Case Manager will assess adult and adolescent clients, coordinate care, and advocate for patient needs while working onsite in Longwood, Florida, with a potential hybrid schedule.
Key responsibilities
- Conduct initial and ongoing assessments of clients, including medical history and treatment needs
- Obtain referrals from behavioral health providers to evaluate client requirements
- Facilitate family meetings and provide outreach and education at community events
Required qualifications
- Bachelor's degree in psychology, social work, or a healthcare-related field
- One year of experience in case management or related patient care support role
- Valid Florida driver's license with a clean driving record
- Bilingual in English and Spanish preferred
