Florida Licensed Case Manager RN

Job Summary

Coordinating patient care from admission to discharge, the full-time Florida Licensed Case Manager RN will monitor care, act as a liaison between patients, families, and healthcare personnel, and develop discharge plans to ensure optimal patient outcomes in a remote setting.

Key responsibilities

• Identifies patient risk factors and evaluates the plan of care through chart reviews and patient interviews
• Educates patients and families on care progression while facilitating communication between all parties involved
• Facilitates ongoing insurance authorizations and ensures compliance with regulatory policies

Required qualifications

• Bachelor's Degree in Nursing
• Florida Registered Nurse (RN) License
• Minimum of 2 years of relevant experience in case management or healthcare
• Knowledge of insurance reimbursement policies
• Experience in developing discharge and transition plans that meet patient needs
