Job Summary
Coordinating interdisciplinary care plans, the full-time Florida Licensed Case Manager RN will work remotely to monitor patient care from admission to discharge, acting as a liaison between patients, families, and healthcare personnel while ensuring compliance with medical advice and insurance requirements.
Key responsibilities
- Identifies patient risk factors and evaluates care plans through chart reviews and patient interviews
- Educates patients and families on care progression and facilitates ongoing insurance authorizations
- Develops discharge plans that address the physical, social, and emotional needs of patients
Required qualifications
- Bachelor's Degree in Nursing
- Current Registered Nurse (RN) license
- Minimum of 2 years of relevant experience in case management or nursing
