Job Summary
Coordinating the overall interdisciplinary plan of care for patients, the full-time Florida Licensed Case Manager RN will work remotely to monitor care, act as a liaison between patients and healthcare personnel, and develop discharge plans that ensure compliance with medical advice.
Key responsibilities
- Identifies patients' risk factors and evaluates the plan of care through chart reviews and patient interviews
- Educates patients and families on care progression while facilitating communication between patients, families, and healthcare personnel
- Facilitates ongoing insurance authorization and maintains knowledge of reimbursement policies to ensure quality care delivery
Required qualifications
- Bachelor's Degree in a relevant field
- Current Registered Nurse Licensing (RN) in Florida
- Minimum of 2 years of relevant experience in case management or a similar role
