Florida Licensed Chronic Care Nurse

Job Summary

To support chronic care management, the remote Florida Licensed Chronic Care Nurse will conduct patient outreach calls, manage a caseload of 275 to 300 patients monthly, and facilitate medication management and patient education.

Key responsibilities

• Conduct approximately 50 to 60 patient outreach calls daily
• Support medication refills, care-plan updates, and physician escalations through clinical conversations
• Utilize personal or company-provided equipment while ensuring security software compliance

Required qualifications

• Active Florida RN licensure with recent certifications and licenses
• Experience in care management, HEDIS, or high-volume outreach preferred
• Ability to learn multiple EHR systems quickly
• Recent remote work experience is advantageous
• Background screening and credentialing required within 60 to 90 days
