Job Summary
To support Medication Therapy Management (MTM) initiatives, the part-time temporary Florida Licensed Clinical MTM Pharmacist will conduct outbound patient consultations, perform Comprehensive Medication Reviews (CMRs), and ensure compliance with Medicare Part D Star Measures while working remotely.
Key responsibilities
- Conduct outbound patient calls regarding MTM services and perform medication consultations
- Complete Comprehensive Medication Reviews (CMRs) and document clinical findings accurately
- Collaborate with healthcare teams to support quality initiatives related to Medicare Part D Star Measures
Required qualifications
- Valid Pharmacist License in Florida (FL)
- Doctor of Pharmacy (PharmD) degree
- Minimum 4 years of experience as a Clinical MTM Pharmacist
- Experience conducting Medication Therapy Management (MTM) services and completing CMRs
- Familiarity with Medicare Part D Star Measures and clinical guidelines
