Florida Licensed Clinical Pharmacist
Job Summary
To support innovative clinical pharmacy services in ambulatory care settings, the part-time Florida Licensed Clinical Pharmacist will provide comprehensive medication management, patient education, and collaborative practice services while working in a hybrid environment of remote and in-clinic patient care.
Key responsibilities
- Provide comprehensive clinical pharmacy services, including medication therapy management and education
- Collaborate with healthcare providers to identify medication-related problems and optimize patient care
- Perform medication reconciliation and monitor patient compliance to improve medication adherence and safety
Required qualifications
- Pharmacy degree (Bachelor of Science or Doctor of Pharmacy) is required
- Minimum of 2 years of experience in clinical practice or specialty pharmacy, or completion of an ASHP-accredited residency
- Must be licensed by the Florida Board of Pharmacy; license must be current and unrestricted
- Experience in Ambulatory Pharmacy or Specialty Pharmacy highly preferred
- Willingness to obtain Florida Consultant Pharmacist licensure within the first 30 days of employment if not currently licensed
