Florida Licensed Clinical Pharmacist

Job Summary

To enhance patient care, the remote Florida Licensed Clinical Pharmacist will provide integrated support to patients and physicians, focusing on medication adherence, clinical protocol development, and monitoring patient outcomes.

Key responsibilities

• Review and interpret clinician's notes for prior authorization approvals and care plan recommendations
• Monitor patient adherence and evaluate therapies for side effects, recommending changes to prescribers as necessary
• Conduct medication therapy management and clinical interventions, including comprehensive consultations and motivational coaching

Required qualifications

• Doctor of Pharmacy (PharmD) or equivalent advanced degree
• Valid Pharmacist license in the state of Florida
• 1+ years of experience preferred but not required
• Proficiency in Microsoft Office tools and database applications
• Knowledge of complex/chronic disease states and therapies used within specialty pharmacy preferred
