Florida Licensed Clinical Pharmacist
Job Summary
To support a Clinical Engagement Services Center, a Florida Licensed Clinical Pharmacist residing in the Sunrise area will conduct telephonic consultations, perform comprehensive medication reviews, and communicate clinical recommendations while working remotely on a full-time basis with rotating shifts between 9:00 AM-8:00 PM CST.
Key responsibilities
- Conduct telephonic consultations and comprehensive medication reviews with members
- Review medications and clinical information to identify gaps in care and therapeutic opportunities
- Provide patient education on medications and document patient care interventions accurately
Required qualifications
- Must reside in the Sunrise, Florida area
- Active and unrestricted Florida Pharmacist license
- Strong knowledge of disease-state management and the 5 Core Elements of MTM Services
- 1+ year of experience working remotely/independently in a cross-functional environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
