Job Summary
To support a pioneering healthcare organization, the part-time or full-time Florida Licensed Clinical Pharmacist will conduct medication therapy management, perform comprehensive medication reviews, and deliver consultations, all while working remotely on a temporary basis.
Key Responsibilities
- Conduct essential outbound phone calls to engage patients in medication therapy management services
- Perform comprehensive medication reviews to ensure patient safety and optimal treatment plans
- Deliver in-depth consultations on statin medication use, aligning with established guidelines
Required Qualifications
- Current and valid Pharmacist's License for the State of Florida
- At least 4 years of clinical experience in medication therapy management
- Demonstrated understanding of pharmaceutical and medical vocabulary
- Proven clinical experience in medication review
- Familiarity with driving key performance indicators related to medication adherence and safety
