Florida Licensed Clinical Pharmacist
Job Summary
To support Medication Therapy Management (MTM) initiatives, the part-time Clinical MTM Pharmacist Medicare Part D will conduct outbound patient consultations, complete Comprehensive Medication Reviews (CMRs), and ensure compliance with clinical guidelines while working remotely.
Key responsibilities
- Conduct outbound patient calls regarding MTM services and perform CMRs
- Provide medication consultations and document clinical findings within web-based systems
- Collaborate with healthcare teams to support Medicare Part D Star Measures initiatives
Required qualifications
- Active Pharmacist License in the State of Florida
- Doctor of Pharmacy (PharmD) degree
- Minimum 4 years of experience as a Clinical MTM Pharmacist
- Experience conducting Medication Therapy Management (MTM) services
- Familiarity with Medicare Part D Star Measures and Comprehensive Medication Reviews (CMRs)
