To support a growing behavioral health care team, the full-time Licensed Clinical Social Worker will engage in care coordination, implement SBIRT, and collaborate with multidisciplinary teams while working in a hybrid remote environment in Broward County.
Key responsibilities
- Support clinical care team with SBIRT implementation and training
- Engage in care coordination to connect members with mental health services
- Conduct Social Determinants of Health screenings and provide community outreach
Required qualifications
- Master's degree in social work (MSW) from an accredited institution
- State Licensure as a Clinical Social Worker
- Specialized certification as Licensed Clinical Social Worker (LCSW)
- 1-3 years of post-Bachelor's experience in mental and behavioral health
- Knowledge of community resources related to mental health services
