Florida Licensed Health Coach
Job Summary
To support a growing integrated health center team, the full-time Florida Licensed Health Coach will provide personalized coaching and group programming to help patients manage chronic conditions and improve overall health, working in-person Monday to Thursday and remotely on Fridays.
Key responsibilities
- Conduct individual and group coaching sessions to promote behavior change and chronic condition management
- Collaborate with healthcare providers to deliver chronic condition coaching based on clinical guidelines
- Facilitate health promotion programs and assist patients with scheduling medical appointments and referrals
Required qualifications
- Bachelor's degree in health science, counseling, psychology, nutrition, social work, or a related field; Master's degree preferred
- Certification from the National Board for Health and Wellness Coaches (NBHWC) required
- Minimum of 2 years' experience as a health coach, including one-on-one and group coaching
- Strong background in health promotion and behavior change methodologies
- Technical expertise in Electronic Medical Record Software and Microsoft 365 applications
