Job Summary
To support injured workers through the coordination of post-acute care services, the full-time Florida Licensed LPN Coordinator will collaborate remotely with healthcare providers, case managers, and insurance professionals, ensuring effective care transitions and positive outcomes while working Monday through Friday from 12:00 PM to 8:30 PM Eastern Time.
Key responsibilities:
- Process referrals for injured workers and obtain necessary clinical information from various sources
- Ensure consistent communication with clients regarding the progress of their injured workers
- Collaborate with case managers and healthcare providers to facilitate appropriate care placements and monitor progress
Required qualifications:
- Current LPN state license in good standing with the State Board of Nursing
- Minimum of one (1) year of nursing experience
- Knowledge of injured worker's rights and commitment to maintaining confidentiality of Protected Health Information (PHI)
- Excellent organizational, prioritization, and multi-tasking skills
- Proficient in medical terminology and computer skills, including Outlook and Teams
