Florida Licensed MTM Pharmacist

Job Summary

To support a dynamic clinical engagement team, the remote Florida Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide disease state management while adhering to HIPAA regulations and organizational policies.

Key responsibilities:

• Engage members telephonically and schedule pharmacist consultation appointments for clinical programs
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens
• Document patient care notes and recommendations while participating in departmental meetings to enhance clinical programs

Required qualifications:

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely/independently in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Min Education: Bachelor's Degree or Pharm D
