Job Summary
To support a leading national healthcare organization, the remote Florida Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and collaborate with healthcare teams on a 6-month contract basis with potential for extension or conversion to full-time.
Key responsibilities
- Conduct telephonic MTM consultations and comprehensive medication reviews
- Identify medication adherence concerns, drug interactions, and gaps in care
- Provide patient education and clinical recommendations while documenting interactions
Required qualifications
- Active, unrestricted Pharmacist license in the state of residence
- Strong knowledge of MTM and disease-state management
- Excellent communication and technology skills
- Ability to work independently in a remote environment
