Florida Licensed MTM Pharmacist
Job Summary
To support a collaborative clinical team, the remote Florida Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document care while working flexible hours in a contract-to-potential-FTE position.
Key responsibilities
- Engage members telephonically to schedule pharmacist consultation appointments and provide clinical recommendations
- Assess medication regimens for efficacy, appropriateness, and adherence while ensuring HIPAA compliance
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
- 1+ years of experience working remotely in a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
