Job Summary

To support patient care coordination, the part-time per diem Florida Licensed Nurse Navigator will serve as the initial contact for patients, ensuring timely appointment scheduling, facilitating communication between patients and healthcare teams, and providing education throughout the care continuum in a remote setting.

Key responsibilities

• Triages new patient appointments and coordinates care by reviewing medical records and scheduling initial visits
• Educates patients and families on treatment plans, disease processes, and available resources while addressing barriers to care
• Collaborates with a multidisciplinary team to ensure continuity of care and tracks patient progress throughout their treatment journey

Required qualifications

• Graduate from an accredited School of Nursing with a BSN required
• Valid State of Florida Registered Nurse (RN) license and BLS certification required
• Minimum of 2 years of relevant work experience in a healthcare setting
• Experience in patient education and care coordination preferred
• Familiarity with oncology care and clinical trials is a plus
