Florida Licensed Nurse Navigator

Job Summary

To support patient care within the health system, the full-time Florida Licensed Nurse Navigator will coordinate initial appointments, facilitate communication between patients and the care team, and assist in navigating the healthcare process remotely.

Key responsibilities:

• Serve as the primary contact for patients, ensuring timely scheduling and coordination of care
• Assess patient needs and facilitate appropriate referrals to support services
• Educate patients and families about treatment options, disease processes, and survivorship care plans

Required qualifications:

• Current Florida RN license
• 3 years of specialty or relevant navigation experience
• Citi Trained Certified or ability to obtain within 3 months of hire
• Experience in patient education and psychosocial support
• Strong organizational skills to manage multiple priorities effectively
