Job Summary
Providing comprehensive clinical management in patients' homes, the full-time Florida Licensed Nurse Practitioner will collaborate with a medical director to deliver patient-centered care, prevent unnecessary emergency department visits, and manage documentation and quality measures remotely.
Key responsibilities
- Deliver in-home clinical medical management, including IV therapy, wound care, and mobile diagnostics
- Participate in on-call rotations to provide after-hours telephonic support to patients
- Document patient conditions and address quality measures to meet or exceed healthcare goals
Required qualifications
- Master's or Doctor of Nursing Practice degree from an accredited Nurse Practitioner program
- Current, unrestricted Nurse Practitioner license and Advanced Practice Registered Nurse licensure in Florida
- National board certification as a Family Nurse Practitioner or equivalent
- Current Drug Enforcement Administration registration and Basic Life Support certification
- Two or more years of advanced practice clinical experience in primary care or related specialty
