Job Summary
To support a growing healthcare team, the remote Florida Licensed Registered Nurse Case Manager will engage telephonically with members, develop individualized care plans, and provide disease management education while ensuring seamless care coordination.
Key responsibilities:
- Engage with members and coordinate their care through telephonic communication
- Develop individualized care plans by compiling clinical information and collaborating with members
- Document care management activities meticulously in the electronic health record
Required qualifications:
- Florida RN license is required
- Associate's of Science in Nursing (ASN) degree is required; Bachelor's of Science in Nursing (BSN) is preferred
- Relevant experience in a healthcare-related field
- Familiarity with community resources and healthcare technology tools
- Experience or willingness to learn about care management within Medicare and Medicaid
