Florida Licensed RN Care Navigator

Job Summary

To support kidney health management, the remote Florida Licensed RN Care Navigator will manage a caseload of patients, providing case management, coordinating care, and educating members on kidney health and treatment options.

Key responsibilities

• Handle inbound and outbound calls to deliver exceptional service to members
• Educate patients on kidney health, dialysis, and transplant options while engaging them in HealthMap's Kidney Health Program
• Collaborate with healthcare providers and support networks to coordinate care and address patient needs

Required qualifications

• Active, unrestricted RN license in Florida
• Bachelor's degree required; 6+ years of RN experience, including 3+ years in case management may be considered in lieu of a degree
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
