Job Summary
To support a multidisciplinary care team, the full-time Florida Licensed Social Work Coordinator will assess patients' psychosocial needs, develop service plans, and coordinate referrals to community resources while working remotely.
Key responsibilities
- Assess patient needs through motivational interviewing and SDOH assessments to create personalized care plans
- Coordinate referrals to internal and external community resources, ensuring closed-loop processes for patient connections
- Maintain accurate documentation of assessments, interventions, and patient interactions in compliance with organizational standards
Required qualifications
- Bachelor's degree in Social Work (BSW) required
- Active social work license
- Minimum of 2-5 years of experience in healthcare, care management, or community-based social work
- Experience addressing SDOH in chronic care or population health settings preferred
