Florida Licensed Telecare Nurse

Job Summary

To support high-volume after-hours patient care, the full-time Florida Licensed Telecare Nurse will triage patient inquiries remotely, utilizing evidence-based practice standards while managing communication through various digital channels.

Key responsibilities

• Conduct after-hours patient triaging and manage inbound and outbound communications for quality patient care
• Assess patient conditions, direct them to appropriate care settings, and develop care plans using Schmitt-Thompson protocols
• Document patient interactions accurately and provide follow-up outreach for symptom management and care coordination

Required qualifications

• Current RN license in the State of Florida
• Bachelor of Science in Nursing (BSN) required within 36 months for new hires
• Three years of clinical experience in a specialty or five years covering multiple service lines
• Ambulatory Care Nursing Certification (AMB-BC) completion required within 36 months of hire
• Experience with Epic EMR and Schmitt-Thompson protocols preferred
