Key responsibilities
- Manage care for patients by completing telephonic assessments and developing tailored care plans
- Communicate with patients, caregivers, and healthcare professionals to provide education and ensure continuity of care
- Document patient interactions accurately while monitoring progress and identifying changes in condition
Required qualifications
- An active and unrestricted Registered Nurse license
- 3-5 years of clinical acute care experience
- At least 2 years of home health experience
- Experience reviewing clinical information and identifying patient needs through nursing judgment
- Experience working with electronic medical records and healthcare systems
