Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time Georgia Licensed Care Manager will develop, assess, and facilitate complex care management activities for members with physical needs in a remote setting.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Develop ongoing care/service plans and collaborate with providers to address unmet needs
- Provide education to members and their families on disease processes and healthcare options
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse state licensure or Compact State Licensure
- Med/Surg experience highly preferred
