Job Summary
To support a growing healthcare team, the full-time Georgia Licensed Care Manager (RN) will develop and facilitate complex care management activities, focusing on personalized care plans and education for members with physical needs, while working remotely from Georgia.
Key responsibilities
- Evaluates member needs and barriers to care, recommending and facilitating plans for optimal outcomes
- Develops ongoing care plans and collaborates with providers to address members' unmet needs
- Coordinates care between members, families, and care providers, ensuring timely access to necessary services
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Georgia RN State Licensure or Compact State Licensure required
- Experience in obstetrics (OB) is essential
- Knowledge of social determinants of health and community resources
