Job Summary

To support a pioneering healthcare team, the temporary Virtual Family Nurse Practitioner will conduct virtual wellness visits, health assessments, and provide comprehensive care coordination while working remotely in Georgia.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits and health assessments
• Collaborate with in-home team members to evaluate patient needs and coordinate care
• Provide comprehensive care management, including preventive care interventions and medication management

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in Georgia
• Experience performing wellness visits
• Technologically savvy with proficiency in using laptops and mobile devices for patient care
• Ability to pass a national background check and valid clinical license search
