Job Summary
To support a diverse member population, the part-time Georgia Licensed RN Case Manager will make outbound and receive inbound calls to assess health statuses, identify treatment gaps, and provide patient education while working remotely.
Key responsibilities
- Make outbound calls and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans and provide referrals to outside sources
- Support members with condition education, medication reviews, and connections to resources
Required qualifications
- Current, unrestricted RN license in the state of residence
- Compact RN license required for those residing in a Compact license state
- 10+ years of experience in case management, hospital, acute care, or direct care settings
- Ability to type and navigate a Windows-based environment
