Job Summary

To provide high-quality, preventive-focused care through telehealth, a Georgia Licensed Telehealth Physician will deliver routine visits to Medicare patients, manage personalized care plans, and collaborate with an integrated care team in a fully remote, full-time position with no on-call or weekend duties.

Key responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions
• Collaborate with the integrated care team to ensure thorough documentation and compliance with quality measures

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license; multi-state licensure is a plus
• Credentialed with major insurance plans such as UHC, Aetna, and Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
