Health Advocate

Job Summary

Working full-time as a Health Advocate on the Healthy Outcomes Team, the candidate will engage in inbound and outbound calls to assist the Medicare population with health screenings and services, fostering healthy behaviors and improving health outcomes.

Key responsibilities

• Conduct inbound and outbound calls to members and providers to facilitate healthcare services, screenings, and medication refills
• Assist members in overcoming barriers to good health while providing education on health decisions
• Document conversations accurately in members' health records to ensure continuity of care

Required qualifications

• 2+ years of experience in a call center or outreach role involving members or consumers
• High School Diploma or GED
• Strong active listening and interpersonal skills
• High computer literacy, including proficiency with Windows and web-based applications
• Reliable high-speed internet connection and suitable remote workspace
