Health Assessor

Job Summary

Engaging with members to conduct health risk assessments, the full-time remote Member Health Assessor will establish professional relationships, coordinate care plan logistics, and verify member eligibility and health status.

Key responsibilities:

• Conduct health assessments by gathering information from members, families, and providers
• Contribute to care plan development and report assessment outcomes to the Care Coordinator
• Assist in providing health education and wellness materials to members

Required qualifications:

• High School Diploma or General Education Diploma (GED) required
• Minimum of two years of experience working with at-risk populations in community settings preferred
• Proficiency in Microsoft Office, including Outlook, Word, and Excel
• Experience working sensitively within diverse cultures
• Ability to maintain confidentiality and act in the company's best interest
