Health Services Assistant

Job Summary

Supporting quality initiatives through data extraction and medical record review, the part-time remote Health Services Assistant will perform chart scrubbing to document compliance with quality measures while collaborating with multidisciplinary teams to enhance patient care coordination.

Key responsibilities

• Researches patient concerns and collaborates with team members to provide resolutions
• Collects and documents ongoing client data in the medical record to support care coordination
• Coordinates multiple appointments and communicates care gaps to the multidisciplinary team

Required qualifications

• High school diploma/GED or equivalent working knowledge
• Knowledge of medical terminology from Medical Assistant, Patient Care Technician, or Certified Nursing Assistant training
• Two years of healthcare experience
• Excellent time management and problem-solving skills
• Strong customer service and recovery skills
